Mental Health Community Residence Program

The Mental Health Community Residence Program provides transitional residential rehabilitation services to men and women, over the age of 18, who are diagnosed with severe and persistent mental illness. Services are tailored to individual needs, but the goal for all clients in this program is to develop skills to live within the community. By actively partnering with East House counselors, family members and therapists, clients lead healthier, productive lives. Many clients who complete our community residence programs transition to our apartment or supported housing programs.
Restorative Services offered through the Mental Health Community Residence Program include, but are not limited to:
- One-to-one counseling
- Financial case management
- Medication management
- Support groups including peer to peer
- Healthy living skills
- Independent living skills
- Vocational assessment and training
- Social outings
Our Mental Health Program Community Residences include:
