Potential Clients
At East House, we are focused on helping adults in recovery from mental illness and substance use disorder to live independent and fulfilling lives. Through a network of services, which includes supported housing and care coordination as well as education and employment programs, East House treats the whole person and helps move individuals from illness to health. If you are interested in East House programs and services, you must have a referral from a community provider.
-Mental Health Residential Program-
If you are interested in East House’s mental health residential program, a complete referral is necessary. All referrals to the East House mental health program are submitted by mental health providers through the Single Point of Access (SPOA) application. -Substance Use Disorder Residential Program-
If you are interested in our substance use disorders residential program, a complete referral is necessary. All referrals to the East House substance use disorder program are submitted by community providers through the Monroe County OASAS Residential Application. -Supportive Housing Services-
If you would like to know more about East House’s supportive housing services, please contact the Admissions Team today at 585-238-4800. Please note: All East House Supportive Housing referrals are submitted by providers through the Single Point of Access (SPOA) application.
