Health Home Care Management


A “Health Home” is a service model whereby all of an individual’s
caregivers communicate with one another so that all of a person’s needs
are addressed in a comprehensive manner. A “care manager” oversees the
process, using health information technology to link services and provides
services including: care coordination, health promotion, transitional
support from one level of care to another, patient and family support,
referral to community and social support services.

East House works with both health homes in Monroe County: Greater
Rochester Health Home Network (GRHHN) and Health Home of Upstate
New York (HHUNY). The goal of the East House Health Home Care
Management Program is to ensure that all of a person’s physical,
behavioral health and social service needs are met. East House Health
Home Care Managers coordinate care and communication among
providers and offer support during transitions in care.
